- Eligibility
- Patients must be enrolled by a provider, give consent, and meet Medicare, Medicare Advantage,
commercial, or self-pay eligibility.
- Patient Responsibilities
- Patients agree to provide accurate information and participate in care coordination.
- Billing
- Services may be billed to insurance or self-pay. Copays and deductibles may apply.
- Intellectual Property
- All content is owned by Healthbridge Solutions.
- Limitation of Liability
- Healthbridge Solutions is not responsible for provider medical decisions.
- Acceptable Use
- Users may not misuse the website or systems.
- Termination
- Services may be discontinued if eligibility is lost or terms are violated.
- Governing Law
- Governed by U.S. federal law and the state of the contracting provider.
